Pharmasists to Refer Scheme

The Vitality Referral scheme is available to people who don’t currently exercise and who suffer from or are at risk of certain diseases that would benefit from physical activity. The prescribed activities are made available to the referred patients at a reduced cost.
Tailor Made
The scheme provides patients with a 12-week activity programme tailored to meet their specific needs. The programme is prescribed by a Health and Physical Activity Officer who also supports patients throughout the 12 weeks along with dedicated Exercise Referral qualified instructors who provide guidance at the activities.
By encouraging pharmacists to recommend Vitality Referral enables them to refer people directly into the scheme and make it easier for people to access the scheme without the need to visit the Doctor or Nurse.
NG9 Centres
In NG9 the Vitality Referral activities take place at Bramcote and Chilwell Leisure Centres and patients may be prescribed activity that sees them participate in the popular Walk and Talk programme, join the Fitness Suite (Gym), take part in instructor led classes and swimming as well.
Vitality Referral is a partnership initiative funded by Broxtowe Borough Council and Nottinghamshire County Teaching Primary Care Trust. The scheme is designed to help people to improve their health by becoming more active.
